SWIMMING TEAM INFORMATION - 2011-12

THE MIDDLE SCHOOL PROGRAM WILL OPERATE AS IT DID LAST YEAR.

ALL 7TH AND 8TH GRADE BOYS AND GIRLS ARE WELCOME TO PARTICIPATE.

6TH GRADERS MAY NOT PARTICIPATE, AND THERE WILL BE NO DIVING.

PLEASE NOTE - THIS IS NOT A LEARN TO SWIM PROGRAM!

The Middle School Swim Season begins the week after Thanksgiving and runs until the last week of January.

A MANDATORY PARENT MEETING WILL BE HELD ON MONDAY, NOVEMBER 28th,

IN THE WBHS MEDIA CENTER AT 5:30 PM - ALL FORMS WILL BE DISTRIBUTED, FILLED OUT, AND

COLLECTED AT THIS MEETING - SO PLEASE BE IN ATTENDANCE

It is not necessary for students to be at this meeting, only parents!

PRACTICE BEGINS ON TUESDAY, NOVEMBER 29TH  FROM 5:00 - 6:15 PM

ALL PARENTS SHOULD BRING TO THE MEETING A COMPLETED PHYSICAL FORM AND

WHITE TREATMENT PERMISSION FORM!

*Blue Physical Form* - No one may practice without this.  Make sure that both you and

your parents have signed the form where required.  If you played in a fall sport, you don't need

another physical.  However, it must  be on file in the High School training room.

If it cannot be located, you will need to have the doctor fill out another form.

Don't delay in taking care of this!  The training room phone # is 865-6741.

*White treatment permission form* - You need one of these even if you played on a

fall sports team.  Make sure that all information is filled in (especially insurance

policy numbers), and that your parent has signed the form.  If you don't have health

insurance,  parents must purchase this through the school.  Forms for this

coverage are available in the High School training room.

DO NOT TURN IN PHYSICAL CARDS OR TREATMENT CARDS TO YOUR MIDDLE

SCHOOL OFFICE!  BRING THEM WITH YOU TO THE POOL OR PARENT'S MEETING.

BLUE AND WHITE FORMS ARE AVAILABLE AT YOUR MIDDLE SCHOOL OFFICES OR AT THE HIGH SCHOOL ATHLETIC OFFICE!

Practice Schedule - This is posted on our website (lakeraquatics.com).

As a rule, we practice Monday - Thursday each week from 5:00 - 6:15 PM, except when there is a HS meet.

Parents are welcome to stay and observe practices from the pool balcony anytime the need arises.

Order Form for Team Apparel - Please bring your checkbooks to the meeting so orders can be

placed right away.

Students cannot participate in practice or meets until this has been done.  The coaches must get verification

from the Athletic office that fees have been paid.
